Broadband usage survey - December 2023

Chorus New Zealand Limited (Chorus, we, our) is the promoter of this survey incentive offer (Offer). 

By providing your details and completing this survey, you acknowledge that:

  • you have read and accepted these terms and conditions, and
  • you authorise Chorus to collect the information you provide. We’ll treat that information in accordance with our Privacy Policy, which may also include us or our partners contacting you with future updates that are relevant to you.

Who is eligible

This Offer runs from Wednesday 29 November to 11.59pm on Wednesday 20 December 2023, so you have until then to complete the survey.

To be eligible, you must:

  • live in New Zealand; and
  • be aged 18 years or over
  • have received an email invitation directly from Chorus to participate in this survey
  • be connected or have been connected to a fibre broadband plan within the past 12 months.

Only one completed survey per physical address and per email address will be accepted and eligible for a $50 Prezzy Card.

How to participate

To participate in this Offer:

  • We’ll send you an invite for the survey: You will have received this survey through an email from Chorus.
  • You complete the survey within the dates specified above: You must have completed the survey, providing a valid address, by 11.59pm 20 December 2023.
  • We’ll confirm a Prezzy Card is on the way: We'll send you an email with details on when your Prezzy Card is on it’s way.

Receiving and registering your card

We will notify you via email that a Prezzy Card is on its way and will courier the Prezzy Card to your address. Please note this will take place in mid-January 2024.

Your card may take up to 21 days to be delivered to you. Depending on the location of the address, or other disruptions, delivery may be further delayed. If you do not receive your card, please get in touch with us.

Once you receive your card, we strongly recommend that you register your card online, at You will also need to unlock it. For full details please refer to the Prezzy Card terms and conditions.

Chorus is not responsible for replacing your card if it is lost or stolen.

General terms

  • Changes and cancellation: Chorus may modify these terms and conditions at any time without notice and reserves the right to cancel, suspend or change the Offer for any reason (including, for completeness, where there is suspected fraudulent or invalid participation).
  • Liability: To the extent permitted by law, Chorus and any other person or party associated with this Offer, will not be liable to you for any direct, indirect or consequential loss or damage, as a result of, or arising from, your participation in the Offer.
  • Platforms: You must comply with the terms of any platform used to participate in the Offer. Chorus and any other person or party associated with this Offer is not responsible for any problems or technical malfunction of any platform, website service, equipment or programme used in or associated with this Offer.
  • Invalid claims: Any request to enter or claim in relation to this Offer after its expiry will be deemed invalid.  Chorus is not responsible for survey responses that are not received, and any incomplete, illegible or fraudulent survey responses are not valid. 
  • Governing law: This Offer is governed by New Zealand law.
